WVPHN Partnership 2025/26
Access Your Supports has partnered with Western Victoria Primary Health Network to deliver the Primary Care Enhancement Project.

This project functions as part of the National Roadmap for Improving the Health of People with an Intellectual Disability and aims to provide tools, training, and resources to general practitioners and clinics to improve their service to people with an intellectual disability. People with intellectual disability face significant barriers in accessing adequate healthcare and currently have a life expectancy up to 27 years shorter than the general Australian population. [1]
A small team at AYS have used their experience in the disability sector to develop the resources below, as well as delivering in person training and support to general practitioners in order to create more inclusive and equitable standards of primary care.
