Access Your Supports is a Geelong based organisation that provides support and services to eligible participants of the National Disability Insurance Scheme (NDIS). With offices in Geelong, Colac and Mildura and over 190 employees, AYS is dedicated to ensuring participants have choice and control to best assist them to achieve their NDIS goals.
AYS Director Ralph Menchise has brought together a team of experienced professionals who are passionate about positive participant outcomes. Not only does Access Your Supports deliver quality support services and empower participants to achieve their life goals with the services available to them under the NDIS, Access Your Supports will also work closely with participants to implement a plan that will work.
ABOUT THE ROLE
Our services aim to assist participants through evidenced based therapy interventions. The program supports participants with approved NDIS Plans.
The Early Intervention Key Worker position is currently viewed as best practice in early childhood intervention, the transdisciplinary model is a collaborative approach that gives participants access to a range of specialists, but therapy is delivered in a personalised way through one qualified professional, the key worker.
Key Responsibilities:
- Provision of therapy in a family-centred approach using a trans-disciplinary model.
- Develop, set and review a Family Services Plan for each Participant annually.
- Draw on the expertise, knowledge and skills from within the team and deliver trans-disciplinary expertise within the Participant home environment, education environment or another place where the Participant spends time
- Draw on more specialised expertise, liaise in greater detail with the relevant professionals.
- Compilation of reports and letters for NDIS Participants, other health professionals and NDIS reporting requirements.
- Maintain records, following company policy and within expected timeframes that clearly document activities undertaken and ensure all documentation is comprehensive, factual and complies with relevant legislation, professional guidelines and organizational policies
ABOUT YOU
The successful candidate will have:
- Tertiary qualification with in Social Work, Special Education, Early Childhood Education, Psychology, Occupational Therapy, Physiotherapy, or Speech Pathology
- Current VIT registration
- Effective time management skills
- Excellent clinical skills
- The ability and initiative to work autonomously while being part of a clinical team
- Demonstrated evidence of clinical supervision and ongoing professional development
- General knowledge and understanding of the National Disability Insurance Scheme (NDIS)
- Alignment with the core business values of Empowerment, Integrity, and Respect.
